Then keep left at the fork, follow signs for Century Blvd and merge onto W Century Blvd. Next, use the right lane to take the I-405 N ramp to Sacramento. Keep left at the fork, follow signs for Interstate 405/Santa Monica and merge onto I-405 N. After that merge onto I-405 N. Keep left to stay on I-405 N, follow signs for Sacramento and use the right 3 lanes to merge onto US-101 S toward Los Angeles. At this point use the 2nd from the left lane to continue on CA-134 E and follow signs for Pasadena. Finally, take the exit toward Vineland Avenue and turn right onto Riverside Dr. Turn right and Performance Care Sports Medicine & Rehabilitation Center will be on your right.
